Treatment of Premature Ejaculation: When we have problems with ejaculation, most of the times we do not seek help. Sometimes we believe the problem would go away on its own, or may be other times we just try to get busy with other things in life and not pay much attention to it. So once, you suspect that you may be having premature ejaculation, book an appointment with your urologist and talk to him. Procrastinating will not help with the problem, when you actually talk about your problems with an expert, he will guide you with the best possible resources. A lot of patients get treated within no time when they get proper treatment as early as possible.

What are the Tests conducted for Premature Ejaculation?

Diagnosis of Premature Ejaculation is mostly clinical. This means, in majority cases, you do not need any major or expensive tests in order to make a diagnosis of premature ejaculation. A consultation session with a good urologist is enough to come to a conclusion. Urologist who is an expert in issues related to men’s health can ask you questions, some of which can be as follows:

  1. How often or how frequently does Premature ejaculation happen?
  2. For how long have you been having problems with ejaculation?
  3. Does this problem happen with one partner or with any other partner?
  4. Has this problem affected your sexual activity?
  5. Does this happen while performing different forms of sexual activity like intercourse, masturbation, visual stimulation etc.?
  6. Do you also have problems related to erection?
  7. Have you been on any medication or recreational drugs recently?
  8. Is this problem affecting your relationship?

Lab testing is usually avoided unless the urologist suspects an infection. In this case, the urologist can suggest simple blood and urine tests to rule out any infection. Like I mentioned before, the diagnosis of premature ejaculation is mostly clinical and not largely laboratory based.

How is Premature Ejaculation Treated?

I will talk about the treatment of Premature Ejaculation by categorising the treatment under two broad headings-

Non Medical Treatment of Premature Ejaculation

These include exercises, phychological therapy, behaviousral therapy, counselling. I will discuss about them in detail below.

Medical Treatment of Premature Ejaculation

A lot of time, pre mature ejaculation is treated without using any drugs. Also there are no specific drugs which can treat premature ejaculation in one go. Lets discuss this in detail below.

Psychological Therapy for Premature Ejaculation

The aim of Psychological therapy is to reduce psychological or emotional stress that may be caused as a result of relationship issues. For example, patient may have performance anxiety with a new partner – in this case, the doctor will help you overcome your anxiety by talking about it and letting you overcome your fears. If there is a relationship problem, you could be helped with couple’s counselling or a sex therapist when required. A urologist is the expert who can correctly sort the problems and help you find the correct help.

Behavioural Therapy for Premature Ejaculation

Behavioural therapy consists of ways in which you can delay the orgasm. Its basically how to control your feelings, build tolerance in order to make you last longer. Here are the following ways in which behaviour therapy is given:

  1. The Start and Stop Method

    In this method, you or your partner stimulates your penis very close to the point of ejaculation and then stops. Now wait for about 30 seconds until you regain your response control. Now start stimulating the penis again and stop as you are about to ejaculate. Repeat this process 3 times. And the 4th time, you can allow yourself to ejaculate. This “Start and Stop Method” should be followed approximately 3 times a week until you gain more control over your ejaculation.

  2. The Squeeze Method

    In this method, you or your partner stimulates your penis until you are close to ejaculation, and then gently squeezes the head of your penis for about 30 seconds, as you begin to lose your erection. Now again starts stimulation till you’re close to ejaculation. This method again is repeated 3 to 4 times and then then you can allow yourself to ejaculate. The aim of this method is to help you attain more control in delaying your orgasms.

  3. The Distracted Thinking Method

    When you’re being sexually stimulated, try to think of non sexual things. For example, you may think of naming all the members of your favourite club, naming all the players of your favourite team. Anything that distracts you from actively thinking about sexual activity.

Medical Treatment of Premature Ejaculation

Like I mentioned before, there is no targeted drug for treating premature ejaculation. However there are drugs that can help you delay the orgasm. SSRIs are a category of anti-depressants which are used to delay orgasms and help with pre mature ejaculation. Be very careful to never self medicate with these drugs. Always seek help and discuss with your urologist if you’re thinking of taking these medications. A urologist will individualise the treatment and make sure you do not suffer any possible side effects of the medication.

There are some anaesthetic ointments and sprays available which are applied on the penis in order to delay the ejaculation. Here, after application of the cream, you need to wait for 15-30 minutes for the drug to get absorbed, and then wash your penis before having an intercourse or else the drug can also cause numbing of your partner’s private parts.

Also, in some cases, erectile dysfunction can be an underlying or an associated cause with pre mature ejaculation. In these cases, the urologist can suggest drugs that are used to treat erectile dysfunction as well such as Sildenafil, Tadalafil, Avanafil etc.

Is it possible to completely treat Premature Ejaculation?

With the treatments that I have listed above, almost 99% patients are able to fully recover. However, it is important to mention that every patient is different, and while we do our best, individuals differ in their ability to follow the treatment. So my best advise is to keep yourself informed, seek help as soon as possible and do not fall into the trap of quacks. Consult a good urologist and you can be treated as soon as possible.
