Large Kidney Stones Treatment by PCNL at Institute of Urology, Jaipur: We report the case of a 55-year-old hypertensive female who presented to the outpatient department at the Institute of Urology, Jaipur, with complaints of intermittent right flank pain. Investigations revealed bilateral renal calculi with mild right hydronephrosis and suspected pyelonephritis. The patient underwent successful right Retrograde Pyelogram (RGP) and Percutaneous Nephrolithotomy (PCNL) with DJ stenting. This case highlights the clinical approach and surgical expertise available at the Institute, particularly under the care of Dr. M. Roychowdhury and Dr. Rajan Bansal.

Introduction: Renal calculi are a common urological concern, often presenting with pain, haematuria, or complications like infection or obstruction. Timely diagnosis and appropriate surgical management are key to symptom relief and prevention of long-term damage. Here, we present a case demonstrating the role of comprehensive evaluation and advanced urological care.
Case Presentation:
A 55-year-old female presented to the OPD at the Institute of Urology, Jaipur, with a history of intermittent right flank pain over the past few days. She was a known case of hypertension, well-controlled on regular medication.
Investigations:
- Ultrasonography (USG): Revealed right renal calculi measuring 15 mm and 16 mm with mild right hydronephrosis. A tiny calculus was also noted in the left kidney.
- Blood Tests: Routine hematological and renal function parameters were within normal limits.
- CT KUB (Non-contrast): Confirmed the findings of right renal calculi and mild hydronephrosis. An additional finding suggested mild right-sided pyelonephritis.
Given the symptomatic presentation, imaging findings, and patient’s background, a thorough discussion was held with the patient and her family. It was decided to proceed with Right RGP and PCNL with DJ stenting.
Management and Outcome:
The procedure was successfully performed by and under the expert supervision of Dr. M. Roychowdhury and Dr. Rajan Bansal, utilizing state-of-the-art infrastructure available at the Institute. The renal stones were completely cleared, and the patient tolerated the procedure well without any postoperative complications.
She was discharged in stable condition and reviewed during follow-up. She reported significant relief in symptoms and improvement in overall wellbeing. A metabolic workup was advised to identify any underlying causes for stone formation.
Discussion:
This case exemplifies the importance of a structured approach — from evaluation to surgical intervention — in managing renal calculi. The presence of mild pyelonephritis added complexity to the case, which was efficiently managed due to the prompt diagnosis and precise surgical planning.
Conclusion:
The Institute of Urology, Jaipur, under the skilled leadership of Dr. M. Roychowdhury and Dr. Rajan Bansal, offers world-class management of all types of renal tract stones. Their combined expertise and experience, along with access to cutting-edge surgical infrastructure, ensures optimal patient outcomes.
